Service Request Forms
Use the following forms to submit request tickets to IT Services:
Employee Network Account Request - Request a GC network account for new employees. Form to be filled out by new employee's supervisor or administrator before employee's start date.
Video Streaming Request - Request livestreaming and/or recording of an event.
Listserv Request - Create a listserv for email subscribers, eliminating the need to type each address in the message header and allowing commenting and responses to other subscribers on related topics.
Banner Account Request - Request an account for Banner/Argos.
