COVID-19 Daily Health Screening

Note: Please see the updated Building Entry Policy. The updated policy notifies faculty and staff entering The Graduate Center building that the health screening form has been replaced by a wellness check survey on Everbridge's health screening app. 

Daily Staff Screening Program:
All staff entering a state owned or leased premise must be screened prior to, or within the first hour of, the start of each shift, and every 12 hours thereafter while on duty. Screening may be performed remotely by staff responding to COVID-19 Daily Health Screening questions below before reporting to office locations.  

Please note that employees of the Advanced Science Research Center (ASRC) do not need to fill out these daily screening questions. Instead, they must follow ASRC’s pre-approved COVID-19 protocols while they are working on the ASRC premises. However, ASRC employees must take Mandatory COVID-19 Response: Return to Work Training and submit their responses to the COVID-19 Daily Health Screening questions below prior to coming to The Graduate Center’s 365 Fifth Avenue building.

After submitting the form, you will receive a confirmation to the email address you submit below. Please keep this email and be prepared to present it if requested.‚Äč


CONTACTS:Have you had any known close contact with a person confirmed or suspected to have COVID-19 in the past 14 days?

TEST RESULTS:Have you tested positive for COVID-19 through a diagnostic test in the past 14 days?

TRAVEL:Have you traveled within a state with significant community spread of COVID-19 for longer than 24 hours within the past 14 days?

TEMPERATURE:Do you have a temperature greater than or equal to 100 degrees Fahrenheit?
Note: Screeners are prohibited from recording employee health data eg temperatures.

SYMPTOMS:Do you have any of the following symptoms (New or Worsening)?
Cough, Shortness of Breath, Troubled Breathing, Fever, Chills, Muscle Pain, Headache, Sore Throat, New Loss of Taste, and/or New Loss of Smell.
