Use the following forms to submit request tickets to IT Services:
- Employee Network Account Request - Request a GC network account for new employees. Form to be filled out by new employee's supervisor or administrator before employee's start date. Refer to the Requesting a Graduate Center Network Account FAQ for help completing the form.
- Software Pre-Procurement Assistance Request - Request assistance with obtaining pricing information for software that is not already provided by the GC or CUNY.
- Video Streaming Request - Request livestreaming and/or recording of an event.
- Listserv Request - Create a listserv for email subscribers, eliminating the need to type each address in the message header and allowing commenting and responses to other subscribers on related topics.
- GC Privileged Access Request - Request assistance with obtaining access to privileged or sensitive GC data.
- Student and Faculty/Staff Laptop Requests - Request a laptop loaner for distance learning & teaching. Laptop request is on first come first serve basis.
- Student Technology Fee Project Proposal Request - Project proposal form that directly benefit GC students
